Northland PACE Senior Care Services is a program designed to keep seniors, ages 55 and over, living independently at home by providing the highest level of care. This includes healthcare, transportation, housekeeping, socialization, and more. As a participant’s needs change, their care plan will change to meet those needs. Services are based on the needs of each individual. Additional services may be necessary to maintain and improve the health of each individual which are determined by the Interdisciplinary Team. If hospitalization or nursing home placement is required, Northland PACE covers the cost, along with the cost of all other healthcare needs. Our team of professionals includes our Medical Director, Nurse Practitioners, Nurses, Social Workers, Occupational and Physical Therapists, health aides and several others who will assist in our participant’s healthcare.
