Social Service Designee


General Description
Performs duties and implements programs that meet physical, mental and psychosocial needs of the residents.  Acts as an advocate of resident's social interest and monitor psychosocial experiences and orientation in the facility.
Essential Duties
Adheres to policies and operating procedures of PCMHA core values and mission.
Ensure medical records are available on admission.
Maintain the quality of each resident's social well being in the facility.
A likeable person with good communication skill.
Analyzing residents needs and mobilizing resources throughout the facility and community to meet residents needs.
Be able to assess situations and individual residents and skillful at conflict resolution.
Assist residents to successfully cope with becoming a resident in the facility and successfully managing the many life adjustments required by this process.
Oversee resident council meeting.
Oversee discharge planning - often observed begins at admission.  Assist in discharge of residents involving family members and community services as appropriate.
Determine when discharge is appropriate.
Adhere to and convey the a philosophy of supporting dignity, privacy, independence, choice and individuality for the residents.
Assist with admission of residents to Wedgewood Manor including readmission or transfers from other areas of the community.  Assures compliance with all regulations regarding admissions and assessments.
Participate in care plan meeting and is responsible for involving residents and family members.
Participate in quality assurance, family and committee meetings.
Obtains resident social histories and evaluates social and family information.
Work closely with the Social Service Consultant.
Perform other duties as assigned by the Administrator.
Work Experience/Skill Set Requirement
Prefer prior experience in Long Term Healthcare setting.
Attention to detail and ability to follow instructions.
Education/Certification/License Requirements
One year of supervised social work experience in healthcare setting, working directly with individuals.
Bachelors degree in sociology, psychology, Gerontology, RN/LPN preferred.
Work Environment/Physical Requirements
Required to perform standing, walking or lifting as needed.
Required to maintain a professional appearance during shift.
