Sheyenne Care Center is a skilled long term care facility licensed for 170 bed. Residents focus is the primary consideration for all employees. The Social Services Department assures that all the residents and their families receive appropriate psycho- social services. This department is the liaison between facility and community agencies in order to assist residents in obtaining needed services. The Social Services Director also provides programs for other departments, residents, and families, and community organizations.

Job Duties:

  • Provide counseling services to residents and families in such areas as interpersonal relationships, stressful life events, disabling diseases, chemical abuse, terminal illness and adjustment to both living in a nursing facility as well as the aging process. The Director uses such techniques as leading groups, individual counseling, and family counseling
  • Organizes and plans programs for the Social Services Department, establishes and updates policies and procedures in accordance with ministry’s policies as well as related local, State, and Federal regulations. This position confers with other departments managers to coordinate programs with their services and needs
  • Participates in residents care plan meetings to develop and assure that department services meet each residents needs
  • Participates in the discharge planning process which includes making transportation arrangements, arranging transfers to hospitals and other facilities, and assisting residents in finding outside services for home living or to other healthcare facilitates
  • Attend necessary meetings
