Content: Effective November 28, 2017, CMS will require nursing facilities to develop a Baseline Care Plan for each resident within 48 hours of admission. This plan should include the instructions needed to provide effective and person-centered care of the resident, AND be presented to the resident/representative in a summary that they understand. So...how do we do that...how do we assure that our process will meet federal mandates, and who is involved?
